Our mission is to deliver the highest quality dental care for you and your family. As a courtesy, we will contact you with a reminder three days prior to your appointment.
If you need to reschedule your appointment, please contact us at least 2 business days before your appointment.
Please keep in mind we do require a confirmation, otherwise you risk forfeiting your appointment. Without advanced notice, we are unable to contact other patients that may have been able to use the time that was reserved exclusively for you. We know emergencies do arise and we will certainly consider your special circumstances. Please keep in mind that multiple cancellations or missed appointments will result in a $50 cancellation fee per hour you are scheduled.
If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
We are committed to the highest level of integrity in the diagnosis and treatment of our patients. Our level of care will not be undermined or influenced by your dental benefit plan. We provide treatment and services necessary to maintain proper oral health regardless of your insurance coverage. Please remember, our agreement for payment is with you, not your insurance provider. To avoid any misunderstandings regarding your dental insurance, we wish our patients to know all professional services rendered are charged directly to the patient and that you are personally responsible for payment fees.
We do not render services on the basis that the insurance companies will pay our fees. As a courtesy our office submits the patient’s insurance claim directly to their insurance company. Any benefit an insurance company pays will go directly to the subscriber. You are welcome to leave your credit card number on file with us for your convenience.
