New Patient Forms

Thank you for choosing our ​office for all your dental needs! We're excited for the opportunity to care for you!  If you are a new patient to our office, the link below contains our new patient registration forms. With everything going on, we do ask the forms be completed prior to your arrival at the office. We are trying to eliminate passing things back and forth. If you have dental insurance, please email a front and back copy of your card to info@gabycosgrovedmd.com

Your initial exam will last approximately one hour. This visit will help us get acquainted with each other and learn about your goals and desires for your dental treatment. Generally, digital radiographs and photographs will be taken allowing your doctor to examine for bone loss, decay, and other pathoses like cysts and tumors.  In some cases, we will also clean your teeth at this visit and provide an evaluation that will outline your existing dental problems and proposed treatment. Oral hygiene instructions will also be provided along with suggestions to help you care for your teeth. In the event a teeth cleaning is performed on the same day, your appointment will be approximately 2 hours.
